I tried 2 diff computers, same issue
fullscreen doesn't solve it
I also tried capping 30 and 60 fps, no changes.

I used to play with glide, I thought about trying it again. I'll give it a go soon.

There is only 1 explanation, you have big % of packet loss, on the way to blizzard servers.

Have same problem and ~ 22-30% packet loss

I don't use glide but I've only heard good things about it so I'd recommend it

use other compatibility ( xp sp3 for exaple )
start as admin
run d2 vid test
check if d2 port is blocked
check if d2 is blocked in firewall
check background processes when running d2

I tried it already
done already
d2 vid test was removed in 1.14
d2 port isn't blocked
d2 isn't blocked in firewall
I did that too, I also tried closing everything but d2 (of course not the windows services)

I had a massive issue like this when i upgraded to windows 10 i moved my diablos across from a hard drive but the lag was so bad i couldnt even play.
found out what was lagging me after a few days.
Windows 10 doesn't need need a compatibility setting set since updates.
right click properties over desktop icon go to compatibility and shut it all off
