Not having insurance isnt the same as not having care/dying, and i already rejected your premise above.
what reliable alternatives are there? private charities aren't going to do the job

People need to be punished for wrong choices.
If someone chooses not to have health insurance you don't just go ahead and bail them out anyway.
Otherwise why would anybody ever want health insurance.
Unless you want to hike taxes for a universal health care system because that's the only way that works.
i do love our universal health care system
though at the same time i'm quite authoritarian and i'd get very oppressive to make sure people didn't make so many wrong choices that requires me to bail them out
