I've seen similar happen and it is definitely more likely to happen 1) the more players there are in the group, 2) when the EPs roll high levels/attributes, and 3) when there are more EPs. Solo is a bit of a different story of course. In solo mode I often come across 3 EPs and the highest tier I get out is a lvl 25. The best high tier drop rate I've had so far was from a 2-EP pull at level 106 (no trapdoors) last week with 0 ML. The tiers were 3 x XIV, 5 x VIII to XI, 2 x V, no low tiers. But take that with a grain of salt, because I have also recently done EPs in solo mode >lvl 110 where the highest tier dropped was tier I lol.
As a side note, I also once did 44 EPs solo at level 69 and the highest tier dropped was a VIII
