- it's because russia got partially kicked out, for mostly political reasons: the whole athletism & other endurance sports are cancerized by doping, not only russia.
- theres more money invested in sports in england, not only soccer
- old colonies & english language; a good destination choice for a career champion coming from africa or eurasia.

each country can arrange the ranking in the way they prefer, from golden only vs total medals, or why not from team vs individual sports etc.

no, the population size is always relevant, since the country will concentrate the best athlets it has in his delegation.
about the unlimited athletes question i guess we have to look forward to richest nations with best lifestyle, probably western countries and some others from asia, like korea. Also china too due the gigantic population....
