wait I have investigated that he drove his Ford truck RUM, I think he died on a car crash..

Captain Rumford can be found lying dead at the bridge towards the old ruins.

His ghost/soul will appear in an Act IV instance.

So a bunch of people are finding a ghostly fake of Captain Rumford as the third Silver Spire ghost (talking to him spawns Azmodan's aspect). Most find themselves wondering "Huh, did he die?", and the conclusion that he's alive but Diablo is trying to mess with you is not uncommon. However he is dead; it's just practically hidden from the player.

After you find Maghda in Wortham and win that fight, before talking to the villager, if you TP back to New Tristram, Rumford walks sideways (lol) past you into the inn before vanishing. He cannot be found anywhere in the inn either.

His body appears at the start of the bridge to Old Tristram Road after you meet Maghda in the cellar. Examining it gives a short dialogue that lacks explanation, and it seems none of the NPCs make any mention of him for the rest of the act.

The body disappears when you speak to or walk past Karyna in the Highlands Crossing after the Queen Araneae fight, and I think that's the last that can be seen of it.

I can't imagine anyone would find out what happens to him naturally. The only time you visit town in the tiny window his body is visible, you arrive and depart by waypoint or portal (or the boat, but that uses the entrance at the other side of town) so you basically have to purposefully avoid progressing in order to find it.

Truth is Rumford never survived the first attack you hear about, he never came back.

What you see from the start posing as Rumford is actually Magdha in disguise.

Thats how she knows who and when to strike when she grabs the stranger, the sword and Leah.

They probably just forgot to tell us about it. Thats also why you find his corpse out in the wilderness when he's supposed to be sitting in town.
