Exactly what happened? was it a certain mob?

I died once and it counted for 3 , went from 36deaths to 39. lost 3x the profs .

its not the first time it happens it me , but usually is 2 deaths.

was a regular mob , could not tell the name tho!

Try and keep track of these kinds of things.
What mob, What char, Etc.

were there 3 mobs or only 1 when u died?

2 mobs if i remember correctly.

Happened to derduff as well with double death against 6 eps in a 3 Men group

I'm thinking,unless someone has something to show otherwise, that taking a hit that gives u 0 life kills u, another hit at same time or millisecond after kills you again and sends you to town, or some sort of thing where two hits at same time go through and result in double or triple deaths.

In happen in run , but in skills too.

You got a lag and lose the item , even if nothing happen or don't click lol.

Really strange.
