Nah, Kenpachi is in SS. I'm going with Ichigo's dad. I think at one point he wielded great power, and maybe even had the Final Getsugatenshou but I'm not sure.

Not sure cause she was being held captive in the real world, but possible.
I'm thinking another member of the espada who stayed alive. Perhaps Grimmjow. He'd have no trouble attacking indisciminately. And we didn't exactly see him die and there was no mention of him dying off-screen either.
